VII. Cause of the accident
1. Direct cause: During the maintenance work, high-temperature sparks were generated when cutting the switch cabinet, causing the cable to catch fire. This is the conclusion drawn based on the oral statement of an on-site personnel. Of course, there may be other causes of the fire, but they are not important.
2. Indirect causes: The cable is not flame retardant, the construction site is chaotic, the operators are unqualified, the cutting operation is not properly controlled, no fire prevention measures are taken, the risk assessment is insufficient, the person in charge of the work does not perform the supervisory role, there is no fire extinguisher on site, the operators are not skilled enough, the escape door of the distribution room is opened inwards, and it cannot be opened due to high temperature, etc.
In fact, the main cause of the fire was that the cable was not flame retardant.
